Case Management Services

Case Managers at ACC Community Connections are responsible for community-based case management services grounded in the dual philosophies of best practice recovery methods and community safety.

Persons found Not Guilty by Reason of Mental Disease or Defect (NGI) can be placed in the community by the courts. The Conditional Release (CR) Program in the Wisconsin Department of Health Services (DHS) is responsible for providing treatment and supervision to these individuals so they can live safely and productively in the community.

Case Managers focus on the client and their needs by putting the client in charge of defining the direction for their lives. Case Managers develop a “toolbox” of methods and resources with the client that enables the client to choose their own pathway to success; the case manager helps them to figure out where they want to go and how to best get there.

Our case management team is based out of offices in Fond du Lac and Appleton providing services in 17 counties, including Adams, Calumet, Columbia, Dodge, Fond du Lac, Green Lake, Kewanee, Manitowoc, Marquette, Outagamie, Ozaukee, Portage, Sheboygan, Washington, Waushara, Waupaca, and Winnebago Counties.

Conditional Release

The Conditional Release (CR) Program in the Wisconsin Department of Health Services (DHS) is responsible for providing treatment and supervision to these individuals so they can live safely and productively in the community.

Opening Avenues to Re-entry Success

The OARS program is a joint venture between the State of Wisconsin’s Department of Corrections (DOC) and Department of Health Services (DHS). The purpose of the program is to fund, coordinate, and administer quality reentry services to the highest need and risk-based population of mentally ill offenders as they prepare for their release from prison and transition to the community.

Outpatient Competency Restoration Program

Individuals charged with crimes must be competent to understand the court process and the possible outcomes of that process. This program offers a least restrictive and cost effective alternative for defendants adjudicated ‘Not Competent’ but ‘Likely to Regain Competency’ and who do not appear to require the full structure, services, and security of an inpatient setting to complete remediation treatment.

Private Case Management

There are times when the mandated provider of case management services is unable to do so because of staffing or geographical issues. ACC has provided case management to a variety of counties and non-profit agencies since 2000.

There are also times when families need case management services. Our staff assists family members with sorting out available resources and finding solutions.
